As a selected model to participate in the "Mrs. Kay's Glamour Gala Fashion Show"
on DATE OF SHOW- I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING AND/OR
VOLUNTEERING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any
risks that may arise from negligence or carelessness on the part of the persons or entities being
released, from dangerous or defective equipment or property owned, maintained, or controlled by them,
or because of their possible liability without fault.
I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or event,
and have not been advised to not participate by a qualified medical professional. I certify that there are no
health-related reasons or problems that preclude my participation in this activity or event.
I acknowledge that the event holders will use this Accident Waiver and Release of Liability Form,
sponsors, and organizers of the activity or event in which I may participate, and that it will govern my
actions and responsibilities at said activity or event. In consideration of my application and permitting me to
participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin,
successors, and assigns as follows:
(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability
arising from the negligence or fault of the entities or persons released, for my death, disability, personal
injury, property damage, property theft, or actions of any kind which may hereafter occur to me including
my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Mrs. Kay's Bridals or
Mrs. Kay's and/or their owners, producers, directors, staff, Event venue, Rental Studios used for fittings
or rehearsals, officers, employees, volunteers, representatives, and agents, the activity
or event holders, activity or event sponsors, activity or event volunteers;
(B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in
this paragraph from any and all liabilities or claims made as a result of participation in this activity or event,
whether caused by the negligence of release or otherwise.
I acknowledge that the Mrs. Kay's Bridals –Mrs. Kay's. and their directors, Event venue, officers,
volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures
to act of any party or entity conducting a specific event or activity on behalf of the Mrs. Kay's Bridals –
Mrs. Kay's,
I acknowledge that this activity or event may involve a test of a person’s physical and mental limits and
may carry with it the potential for death, serious injury, and property loss. The risks may include, but are not
limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment,
vehicular traffic, and actions of other people including, but not limited to, participants, volunteers, spectators,
coaches, event officials, and event monitors, and/or producers of the event, and lack of hydration. These
risks are not only inherent to participants but are also present for volunteers.
The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such
capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such
The Producers will make the final decision regarding any production or operation of the show.
The Producers reserve the right to amend/modify model line up to any designer at any time prior to or
during the show. Anyone verbally/physically abuse staff will result in the immediate dismissal of the show
and escorted by security. Any physical /verbal altercation or abuse will result in the immediate dismissal of the parties involved regardless of any explanation or reasoning of the altercation. Parties will be dismissed from the event and escorted by security. Mrs. Kay's , the show’s venue, sponsors, or partners will not be responsible for the loss/damage items. We do ask all participants to keep valuables at home.
LEGAL NOTE: Venue security cameras within the venue will be used to identify then arrest and prosecute any model/individuals caught executing a theft/damage to venue and/or personal items and/or property – it will also result in the dismissal from the show event and escorted by security. All models must appear at the show venue on time for call time at noon Sharp for check-in.
All models must advise their guest(s) of the show’s dress code required to attend our show. If a fashionable dress code is not followed then regardless of purchase of admission show seat – the guest will not be admitted. All guests (s) are to dress fashionably. Sneakers, fitted caps, shorts, athletic wear, sloppy attire and jerseys are not acceptable. All guest(s) must check their coats at the coat check area (no exceptions). Only models in an upcoming fashion show are allowed in the backstage area. Models are not allowed any additional guest(s) backstage (including additional help, family, friends, etc.)
ALL MODELS MUST ARRIVE TO THE SHOW VENUE IN FITTED SKINNY BLUE JEANS, FITTED
WHITE TOP WITH VEST OR BLAZER JACKET AND VEST WITH HEELS/DRESS SHOES MEDIA-READY FASHIONABLE
ATTIRE AND PREPARE FOR INTERVIEWS AT OUR RED CARPET MEDIA WALL (CAPS, BAGGY
JEANS, JERSIES, BIG T-SHIRTS, SWEATS ETC ARE NOT ALLOWED).
MODELS WILL BE DISMISSED IMMEDIATELY FOR BRINGING UNAUTHORIZED INDIVIDUALS OR
FOOD OR BEVERAGE IN ANY FORM BACKSTAGE.
IMPORTANT DESIGNER/MODEL FITTING DATE – ALL MODELS MUST ATTEND THE
DESIGNER/MODEL FITTING TO CONTINUE TO BE IN THE SHOW - MANDATORY TO ATTEND –
STUDIO LOCATION -642 Hilliard Mills Manchester CT 06042, March 9th at 12 pm
SHOW SEATS ARE LIMITED
Models and/or parent or guardian of models under 18 years of age must sign and submit all model
“Required Forms” in order to participate in the show. Models must submit these forms prior to the
show date. If all models’ “Required Forms” are not submitted to the show’s producers then the
model will be dismissed immediately from the show. These “Required Forms” are; Accident Wavier,
Photo/Video/Audio Release, Parental/Legal Guardian Consent – only required if the model is less than 18
years of age and Code of Conduct. Models will be dismissed immediately – without explanation – for
brining unauthorized individuals backstage.
FOOD AND BEVERAGES NOT ALLOWED ANY GUEST(S) BACKSTAGE OR WITHIN THE EVENT
VENUE. ANY MODEL FOUND WITH FOOD AND BEVERAGES AND EATING BACKSTAGE OR WITHIN
THE EVENT VENUE WILL BE DISMISSED FROM THE SHOW AND REMOVED FROM THE VENUE.
